Good speeds/connection initially, now incredibly slow

I recently moved about a month ago and had my service upgraded to Performance (internet plus basic cable package).  A Comcast tech was here and checked the line during setup. 

 

My speed and connection were great the first month, but over the last several days I've been getting less than 1 mbps downstream and 1-3 mbps upstream pretty consistently. I have a DOCSIS 2.0 modem that's about four years old. The diagnostic utility is showing a SNR of about 30 (range between 26 and 32). The signal on my TV is fine though. Could the modem be the problem?
