Cable Modem: Arris TM402P/110 (Leased)

Unable to log into modem to view modem diagnostics, have tried following addresses:
Timeframe: Within past 5-7 days
Initial incident: Modem reboots, UL/DL lights blinking simultaneously, as if recieving a firmware update, after two subsequent reboots of modem, connection resumes.
Continuing Issues: Since the initial event, I am unable to maintain any constant connection to the internet. At it's worst, I experience delays in both connection and dropping of connections approx 5 times in any given 15 minute interval. At it's best, I experience one disconnect in 15-20 minutes.
Location: Rockford, Illinois
OS: Irrelevant, experienced on several different computers running different operating systems.
Connection type: Both wired (rj45 connection to router) and wireless (2.4Ghz, 5Ghz) have experienced issues
