Unstable internet speeds?

I've been having issues lately with my internet. My speed seems to change alot. I was running a smoothwall for the longest time, but i removed it an went back to a basic E2000 router to see if it was my issue which it is now clear it is not. My family pays for Blast internet + Tv. I should be getting 50 Mb download and 10 Mb upload. Well over the last week my download has been fluctuating from 10~40Mb and my upload which is what I rely on the most has been roughly 1Mb, according to speedtest. I need to figure out whats going on since its this bad. I also used probeshaper and it says im getting alot of high packet loss outgoing and incoming. I have a cable that comes in to the house directly to a 8 way amp now b/c before the signals to the bedrooms were too weak before, as in alot of the tv cutting out, and messages " your service should return momentarily." I tired the online rep service and they keep sending me new modems (which I have to pay shipping for) which obviously is not fixing the issue. I'm suppose to get my fourth one this year. I hardly doubt that it's the hardware but an issue somewhere along the poles, but last time I talked to the rep they couldn't even detect that my modem was online, and the time before that they kept telling me it was de-syncing from the comcast network which is probably a lie. Resetting my modem several times doesn't fix my issues. I have replaced my cable lines in the house twice now with RG6U and Cat5E to ensure there were no breaks in my own home anywhere. I personally refuse to let a tech step into the home now b/c last time they sent someone to the house they replaced the modem, and hung it from the wall by its wires, and it didn't even fix the issue.  Any help is greatly appreciated.
